When to use this form
You should use this form only if your concern relates to a potential breach of the Code, including where a signatory may not have appropriately:
- Addressed online enabled harm experienced through their service (e.g. serious safety-related harm, including sexual misconduct or serious non‑sexual harm)
- Responded to or handled a complaint about such harm
- Provided appropriate outcome communications or support resources following a complaint (where required under the Code)
- Met its obligations under the Code’s safety, moderation or complaint-handling requirements.
Before you submit
This form is not the right channel for issues relating to:
- Your account being banned, suspended, or restricted, including requests for appeals
- Billing, subscriptions, refunds, or technical issues
- General dissatisfaction with another user that does not relate to online enabled harm
- Requests for customer support that does not relate to breach under the Code or breach of the platforms safety policies and procedures
These matters should be directed to the dating service directly.
